Check the PDF on a phone first
An A4 document designed for print is often unreadable on a phone without pinching and dragging. If most scans will be on mobile, a simple web page beats a PDF.

Does the PDF need to be online?
Yes. The QR code stores a web address, not the document itself. The file must be hosted somewhere reachable.
